INSURANCE ELIGIBILITY & DISCOUNT PROGRAMS
At Wesley Health Centers we work with every individual in providing medical care to insured and uninsured patients of all ages. Outreach and Enrollment staff will assist you in applying or renewing AHCCCS (Medicaid) services. If you are uninsured we offer a sliding fee scale (SFS) medical discount program. Your family’s Income and household size is used to determine individual’s sliding fee qualifications. We also assist in applying or renewing SNAP (food stamps) benefits. Required documents to bring in at the time of your appointment are listed below.
AHCCCS/MEDICAID ELIGIBILITY REQUIRED DOCUMENTS
(Provide one of the following)
- Proof of income for past 30 days
- Pay stubs
- Letter from Employer
- Recent Tax Forms
- Child Supports, Social Security, Pension, Unemployment
- Proof of residence
- Rental/Mortgage Statement
- Utility Bill
- Statement from someone outside the home
- Proof of Citizenship or Permanent Residence
- Birth Certificate
- Passport
- Resident Card
- US Naturalized Certificate
- Social Security Card for each member applying
- School Verification (minor children)
- Proof child is enrolled in school
- Proof of income for past 30 days
