NUTRITION EDUCATION TO KEEP YOUR FAMILY HAPPY AND HEALTHY

Who is Eligible for WIC?

You can participate in WIC if you live in Arizona, have a nutritional need (our staff can help you determine this), and you are one of the following:

• Pregnant
• Have delivered a child within the last six months
• Breastfeeding a child within the last year
• Infants and children under the age of five, including foster children

What does WIC offer?

• Nutrition education
• Breastfeeding support
• Experts who specialize in nutrition for mothers and their children
• e-WIC card to buy healthy food
• Referrals to health care, immunizations and other community services

Apply to WIC today!

Submit the necessary documents to get started. Here is what you’ll need:

• your driver’s license or photo ID
• identification for everyone enrolling in the program. For children, that may be a birth certificate, crib card, or shot card/immunization record
• proof of address (ex. Electric bill)
• proof of income (ex. Pay stub or letter from AHCCCS or SNAP)
